You need a Thames Valley League form, Chinnor Youth membership form and a cheque for £30 made payable to 'Chinnor Youth Football Club'

Please note: Don't pay for a passport photograph just use a digital camera and print your own. Proof of age can be a page from your passport/driving license, as long as it shows your DOB and your name/picture. You do not need to pay the registration fee CYFC will pay this for you.

Under 10 – under 16's

You require 2 passport sized photographs, proof of age (photocopy or birth certificate or passport), a completed Oxford Mail Girls Football League and a Chinnor Youth membership form , Parental Consent form and the Code of Conduct and a cheque for £30 made payable to 'Chinnor Youth Football Club'

Currently the 'Parental Consent' form is not available on line – ask your team manager for a copy.
